Medical (including vision and prescription) – Member Insurance offers employees a PPO plan (Choice Plus) sponsored by United Healthcare. Medical benefits are effective the first of the month following the date of hire. The cost to the employee varies depending upon which tier of coverage is needed (employee only, employee + spouse, full family, etc). Member Insurance strives to keep employee paid portions of the medical premiums reasonable and stable while providing the best coverage possible. Employees contribute a portion of their medical premium through payroll deduction.
Dental – Member Insurance offers a dental PPO plan through Principal Life Insurance Company. The cost to the employee varies depending upon which tier of coverage is needed (employee only or family). Employees contribute a portion of their dental premium through payroll deduction.
Short Term Disability (STD) – Member Insurance offers a 100% employer paid premium short-term disability program through Principal Life Insurance Company. This coverage is available to full-time employees scheduled to work at least 30 hours a week and is effective the first of the month following 90 days of employment.
Long Term Disability (LTD) – Member Insurance offers a 100% employer paid premium long-term disability program through Principal Life Insurance Company. This coverage is available to full-time employees scheduled to work at least 30 hours a week and is effective the first of the month following 90 days of employment.
Life and Accidental Death or Dismemberment – Member Insurance offers a 100% employer paid premium plan for life insurance in the amount of two times your annual salary up to $250,000. Additional voluntary Life/AD&D coverage may be purchased by the employee for themselves and their dependents.
Flexible Spending Account/Unreimbursed Medical Expenses – Member Insurance offers a pre-tax savings plan for unreimbursed medical expenses, including vision, dental, doctor visit co-pays, prescription co-pays and some over the counter medications.
Flexible Spending Account/Dependent Day Care – Member Insurance offers a pre-tax savings plan to be used for costs associated with dependent day care (both child and elder care).
Off-The-Job Accident –Member Insurance offers a voluntary accident insurance plan that includes an annual wellness ride through Transamerica. This plan will pay benefits in addition to any other coverage the employee may have in place.
Benefits Continuation – The Federal Consolidated Omnibus Budget Reconciliation Act (COBRA) gives employees and their qualified beneficiaries the opportunity to continue health insurance coverage under Member Insurance’s health plan when a qualifying event results in the loss of eligibility.
